Creating Individual Health Care Plans Online
Using our simple medical condition forms you can create a Department for Education compliant individual health care plan. Photos pull through from your MIS and you can add consent for parents or carers to authorise the IHCP.
Care Plan Review Date Reminders
Receive automated reminders when individual health care plans (IHCPs) are up for review. Simply enter the review dates, usually every 12 months, and set-up automated alerts for specific staff members to receive an email four weeks before it is due for review.
If a parent or carer requests your school to administer prescribed medication you will need them to complete a medication consent form. Using Medical Tracker you can create, or upload, a medication consent form - this will not only keep a historic record but also keep all staff up to date about each student's wellbeing.

Create medication consent forms
Using our simple medical condition forms you can create a Department for Education compliant medication consent form. You can enter the 'Last date medication to be administered' and include parent or carer consent.
If a student has to take medication for a set period the 'Last date medication has to be administered' can be entered into Medical Tracker. Once this date has been reached the medical condition will be automatically archived. This will keep a history of a student's medical conditions within your school.

Daily medication administration register
Create a daily medication register which will tell staff which students need to take their medication that day. This is a great contingency plan if regular staff are not in school. The register includes:

Auto-fill medication administration records
When you need to record medication administration the application will automatically feed through the dosage and medication that have been entered into the medical condition module. This will allow you to simply select ‘Apply’ which will not only ensure the correct medication and dosage is being administered but it will also complete the appropriate fields.
Medical condition alerts
Once a medical condition has been created it will create alerts across the application. This means that if a staff member records an incident for a student with Asthma an alert will automatically pop-up to safeguard the student - great for Ofsted.
You can upload supporting documentation to give staff further insight into a student's medical condition.
Archived medical conditions
Medical conditions can be archived at any time and a full audit trail is kept of who, when and why the condition was archived. This keeps a full medical history for every student.
MIS medication condition sync
If your school uses Capita SIMS, Integris or Bromcom you can integrate the medical condition field with Medical Tracker. This allows Medical Tracker to be updated directly from your MIS.
